Swing bed program available at The University of Kansas Health System

The University of Kansas Health System Great Bend Campus offers swing bed care, a service provided in rural hospitals with a Medicare service agreement. The program allows patients who are ready to move from acute care to skilled nursing care to stay in the same hospital for their care instead of being moved to a nursing facility.

It also allows them to stay closer to home. A specialized care team provides personalized care to help people regain their independence.

People who could benefit from swing bed care include those who are recovering from surgical procedures or receiving IV therapy, physical or occupational therapy or being treated for wound, skin or other medical conditions that need additional care. Others who could benefit from swing bed care are those who need assistance in learning how to manage their medications.

Every person who goes through the swing bed program receives a personalized discharge plan that outlines specific steps for continued success and will ensure a smooth transition to the next phase of their recovery.

Swing bed care requires a physician referral. Care will be coordinated with the person’s healthcare team to ensure a smooth transition.
