Barton County coronavirus update: 67 active cases

LOS ANGELES (AP) — California’s state epidemiologist is urging a halt to more than 300,000 coronavirus vaccinations using a Moderna vaccine version because some people received medical treatment for possible severe allergic reactions.

Dr. Erica S. Pan on Sunday recommended providers stop using lot 41L20A of the Moderna vaccine pending completion of an investigation by state officials, Moderna, the U.S. Centers for Disease Control and the federal Food and Drug Administration.

“Out of an extreme abundance of caution and also recognizing the extremely limited supply of vaccine, we are recommending that providers use other available vaccine inventory,” Pan said in a statement.

She said more than 330,000 doses from the lot arrived in California between Jan. 5 and Jan. 12 and were distributed to 287 providers.

Fewer than 10 people, who all received the vaccine at the same community site, needed medical attention over a 24-hour period, Pan said. No other similar clusters were found.

Pan did not specify the number of cases involved or where they occurred.

However, six San Diego health care workers had allergic reactions to vaccines they received at a mass vaccination center on Jan. 14. The site was temporarily closed and is now using other vaccines, KTGV-TV reported.

Moderna in a statement said the company “is unaware of comparable adverse events from other vaccination centers which may have administered vaccines from the same lot.”

The CDC has said COVID-19 vaccines can cause side effects for a few days that include fever, chills, headache, swelling or tiredness, “which are normal signs that your body is building protection.”

However, severe reactions are extremely rare. Pan said in a vaccine similar to Moderna, the rate of anaphylaxis — in which an immune system reaction can block breathing and cause blood pressure to drop — was about 1 in 100,000.

The announcement came as California counties continue to plead for more COVID-19 vaccine as the state tries to reduce its rate of infection, which has led to record numbers of hospitalizations and deaths.

California, with a population of 40 million, has shipped about about 3.2 million doses of the vaccine — which requires two doses for full immunization — to local health departments and health care systems, the state’s Department of Public Health reported Monday.

Only about 1.4 million of those doses, or around 40%, have been administered.

So far. the state has vaccinated fewer than 2,500 people per 100,000 residents, a rate that falls well below the national average, according to federal data.

Although Gov. Gavin Newsom announced last week that anyone age 65 and older would be eligible to start receiving the vaccine, Los Angeles County and some others have said they do not have enough doses to vaccinate so many people and are first concentrating on inoculating health care workers and the most vulnerable elderly living in care homes.

On Monday, the superintendent of the Los Angeles Unified School District sent a letter to state and county public health officials asking for authorization to provide COVID-19 vaccinations at schools for staff, local community members and for students once a vaccine for children has been approved.

“Doing so will help reopen schools as soon as possible, and in the safest way possible,” Superintendent Austin Beutner wrote.

California is nearing 3 million coronavirus cases and more than 33,600 people have died since the start of the pandemic last year, according to figures from Johns Hopkins University.

The death rate from COVID-19 in Los Angeles County — the nation’s most populous and an epicenter of the state pandemic — works out to about one person every six minutes.

On Sunday, the South Coast Air Quality Management District suspended some pollution-control limits on the number of cremations for at least 10 days in order to deal with a backlog of bodies at hospitals and funeral homes.

“The current rate of death is more than double that of pre-pandemic years,” the agency said.

Health official urges stop to 300K vaccines after some fall ill

Dr. Andrew Carroll — a family doctor in Chandler, Arizona — wants to 
help his patients get immunized against covid, so he paid more than 
$4,000 to buy an ultra-low-temperature freezer from eBay needed to store
 the Pfizer vaccine.

But he’s not sure he’ll get a chance to use 
it, given health officials have so far not said when private doctor’s 
offices will get vaccine.

Not
 only are doctors having trouble getting vaccine for patients, but many 
of the community-based physicians and medical staff that aren’t employed
 by hospitals or health systems also report mixed results in getting 
inoculated. Some have had their shots, yet others are still waiting, 
even though health workers providing direct care to patients are in the 
Centers for Disease Control and Prevention’s top-priority group.

Many
 of these doctors say they don’t know when — or if — they will get doses
 for their patients, which will soon become a bigger issue as states 
attempt to vaccinate more people.

“The reason that’s important is 
patients trust their doctors when it comes to the vaccine,” said 
Carroll, who has complained on social media that his county hasn’t yet 
released plans on how primary care doctors will be brought into the 
loop.

Collectively, physicians in the county could vaccinate 
thousands of patients a day, he said, and might draw some who would 
otherwise be hesitant if they had to go to a large hospital, a 
fairground or another central site.

His concern comes as, 
nationally, the rollout of the vaccine is off to a slower start than 
expected, lagging far behind the initial goal of giving 20 million doses
 before the new year.

But Dr. Jen Brull, a family practice doctor 
in Plainville, Kansas, said her rural area has made good progress on the
 first phase of vaccinations, crediting close working relationships 
formed well before the pandemic.

This fall, before any doses 
became available, the local hospital, the health department and 
physician offices coordinated a sign-up list for medical workers who 
wanted the vaccine. So, when their county, with a population of 5,000, 
got its first 70 doses, they were ready to go. Another 80 doses came a 
week later.

“We’ll be able to vaccinate almost all the health care-associated folks who wanted it in the county” Brull said recently

Dr.
 Jason Goldman, an internist in Coral Springs, Florida, said he was able
 to get vaccinated at a local hospital that received the bulk of 
vaccines in his county and oversaw distribution.

In the weeks since, however, he said several of his front-line staff members still “don’t have access to the vaccine.”

Additionally,
 “a tremendous number” of patients are calling his office because 
Florida has relaxed distribution guidelines to include anyone over age 
65, Goldman said, asking when they can get the vaccine. He’s applied to 
officials about distributing the vaccines through his practice but has 
heard nothing back.

Patients “are frustrated that they do not have
 clear answers and that I am not being given clear answers to provide 
them,” he said. “We have no choice but to direct them to the health 
department and some of the hospital systems.”

Another troubling 
point for Goldman, who served as a liaison between the American College 
of Physicians and the expert panel drawing up the CDC distribution 
guidelines, is the tremendous variation in how those recommendations are
 being implemented in the states.

The CDC <a href="https://www.cdc.gov/coronavirus/2019-ncov/vaccines/recommendations-process.html" target="_blank">recommends</a>
 several phases, with front-line health care workers and nursing home 
residents and staff in the initial group. Then, in the second part of 
that phase, come people over 75 and non-health care front-line workers, 
which could include first responders, teachers and other designated 
essential workers.

States have the flexibility to design their own
 rollout schedule and priority groups. Florida, for example, is offering
 doses to anyone 65 and up. In some counties, older folks were told 
vaccines were available on a first-come, first-served basis, a move that
 has resulted in long lines.

“To say right now, 65-plus, when you 
haven’t even appropriately vaccinated all the health care workers, is 
negating the phasing,” said Goldman. “There needs to be a national 
standard. We have those guidelines. We need to come up with some 
oversight.”

On Thursday, the American Hospital Association echoed 
that concern in a letter to Health and Human Services Secretary Alex 
Azar. Hospitals — along with health departments and large pharmacy 
chains — are doing the bulk of the vaccinations.

Calling for additional coordination by federal officials, the <a href="https://www.aha.org/system/files/media/file/2021/01/aha-hhs-leadership-actions-needed-expedite-administration-covid-19-vaccines-lettetr-1-7-20.pdf" target="_blank">letter</a>
 outlined what it would take to reach the goal of vaccinating 75% of 
Americans by the end of May: 1.8 million vaccinations every day. Noting 
there are 64 different rollout plans from states, cities and other 
jurisdictions, the letter asked whether HHS has “assessed whether these 
plans, taken as a whole, are capable of achieving this level of 
vaccination?”

Lack of direct national 
support or strategy means each county is essentially on its own, with 
success or failure affected by available resources and the experience of
 local officials. Most state and local health departments are 
underfunded and are under intense pressure because of the surging 
pandemic.

Still, the success of vaccination efforts depends on planning, preparation and clear communication.

In
 Lorain County, Ohio, population 310,000, local officials started 
practicing in October, said Mark Adams, deputy health commissioner. They
 set up mass vaccination clinics for influenza to study what would be 
needed for a covid vaccination effort. How many staff? What would the 
traffic flow be like? Could patients be kept 6 feet apart?

“That 
gave us an idea of what is good, what is bad and what needs to change,” 
said Adams, who has had previous experience coordinating mass 
vaccination efforts at a county level.

So, when the county got its
 first shipment of 500 doses Dec. 21, Adams had his plan ready. He 
called the fire chiefs to invite all emergency medical technicians and 
affiliated personnel to an ad hoc vaccination center set up at a large 
entertainment venue staffed by his health department. Upon arrival, 
people were greeted at the door and directed to spaced-apart “lanes” 
where they would get their shots, then to a monitoring area where they 
could wait for 15 minutes to make sure they didn’t have a reaction.

Right
 after Christmas, another 400 doses arrived — and the makeshift clinic 
opened again. This time, doses went to community-based physicians, 
dentists and other hands-on medical practitioners, 600 of whom had 
previously signed up. (Hospital workers and nursing home staff and 
residents are getting their vaccinations through their own 
institutions.)

As they move into the next phase — recipients 
include residents over 80, people with developmental disorders and 
school staff — the challenges will grow, he said. The county plans a 
multipronged approach to notify people when it’s their turn, including 
use of a website, the local media, churches, other organizations and 
word-of-mouth.

Adams shares the concerns of medical providers 
nationwide: He gets only two days’ notice of how many doses he’s going 
to receive and, at the current pace of 400 or 500 doses a week, it’s 
going to take a while before most residents in the county have a chance 
to get a shot, including the estimated 33,000 people 65 and older.

With
 10 nurses, his clinic can inject about 1,200 people a day. But many 
other health professionals have volunteered to administer the shots if 
he gets more doses.

“If I were to run three clinics, five days a 
week, I could do 15,000 vaccinations a week,” Adams said. “With all the 
volunteers, I could do almost six clinics, or 30,000 a week.”

Still,
 for those in the last public group, those age 18 and up without 
underlying medical conditions, “it could be summer,” Adams said.

It’s been a year since the first COVID-19 case was documented in Wuhan, China. Since then, the disease has spread around the globe and throughout the United States. More than 20 million people in our nation have become infected, and more than 350,000 have died.

But there is hope. Vaccines are here, and monoclonal antibody treatment provides a new therapy for patients with mild to moderate illness.

“Believe the science,” says Steve Stites, MD, chief medical officer for The University of Kansas Health System. “It’s not going to lead you astray. Science is bringing us vaccines. Science is bringing us monoclonal antibody treatment. And we will tell you what we know.”

"It has been a long, challenging journey," adds Jonathan Pike, MD, family medicine physician at The University of Kansas Health System Great Bend Campus. "We still face many unknowns with COVID-19. However, as evidenced by vaccines and monoclonal antibody treatments, each new day brings more information and a better understanding of the virus. It shows that our community efforts and sacrifices to practice safety measures have not been in vain."

The situation is a dynamic one and changes rapidly. The University of Kansas Health System has been at the forefront of receiving and sharing important information.

The latest on COVID-19 vaccines Multiple pharmaceutical manufacturers have produced COVID-19 vaccines. They are in various stages of FDA approval. Distribution has begun under emergency use authorization. This is special FDA permission to use new drugs in emergency scenarios to promote public health even while late-stage research continues.

A CDC volunteer team called the Advisory Committee on Immunization Practices (ACIP) has met numerous times to discuss and plan vaccine distribution. The committee determined that the first doses of the vaccine will be given to frontline healthcare workers and residents of long-term nursing care facilities. Production of the vaccine will continue until the general public has access.

The vaccine will be given in 2 doses per person. It may be needed annually, though complete information is not yet known.

What is monoclonal antibody treatment? It sounds a little intimidating, but monoclonal antibody treatment is something to feel good about. It’s new a therapy for people with mild to moderate COVID-19. It is given to select patients who test positive for SARS-CoV-2, the virus that causes COVID-19, and are believed to be at high risk for developing severe illness.

A monoclonal antibody is a medical product produced in a laboratory. These molecules are designed to act just like the antibodies the body would produce to recover from a disease. The laboratory-produced molecules are given to the patient to mimic the immune system’s reaction to disease – in effect, tricking the body into thinking it has COVID-19 and fighting it off. Monoclonal antibodies for COVID-19 – called bamlanivimab – battle the virus, reducing its ability to cause severe illness or damage.

The University of Kansas Health System has received a limited number of bamlanivimab doses. We provide it to eligible patients, who must meet criteria as follows:

· Patients must meet all of these: o Age &gt; 12 years o Body weight &gt; 40kg o Within 7 days of symptom onset o Confirmed COVID-19 with mild to moderate symptoms o Without increased oxygen requirements o Not actively hospitalized and unlikely to require hospitalization due to COVID-19 o Not pregnant or breast feeding o No history of a positive SARS-CoV-2 antibody o Has not received SARS-CoV-2 convalescent plasma or another SARS-CoV-2 monoclonal antibody

· And one of these: o Age &gt; 65 years o BMI &gt; 35 kg/m2 o Chronic kidney disease of stage IV or above, diabetes mellitus, active cancer, solid organ transplant recipient, HIV with inadequate CD4 count, cirrhosis, sickle cell disease or humoral immunity defects (inherited, due to chemotherapy or other immunosuppressant therapy) o Age &gt; 55 years with chronic lung diseases, cardiovascular disease or hypertension o Age 12-17 years with BMI &gt; 85th percentile, sickle cell disease, heart disease, neurodevelopmental disorders, chronic respiratory disease requiring daily medication for control, medical-related technological dependence

Monoclonal antibody treatment is given by infusion on an outpatient basis. It must be given as soon as possible after the positive COVID-19 test result is confirmed, so it’s important to get tested quickly after you notice the first signs or symptoms.

Great Bend physician weighs in on COVID-19 prevention

Governor Kelly announced Thursday her administration’s COVID-19 distribution order for population groups in Kansas. Kelly reported these phase groups were created using guidance from national and state public health experts and input from the Kansas Coronavirus Vaccine Advisory Council (COVAC).

To date, the Barton County Health Department has collaborated with community partners to administer all of the vaccine allotted to the health department at this time. This effort has fulfilled the majority of the KDHE guidance to vaccinate health care workers, emergency responders and other frontline workers as outlined in Phase 1 of the Governor’s plan.

It is anticipated we will receive more vaccines in the near future. We will work diligently to distribute these vaccines following the guidance of KDHE. The health department will be providing future information and instruction to the next vaccine recipients as soon as more doses arrive.

Barton County: COVID vaccination phases by population

TOPEKA, Kan. (AP) — Kansas expects to finish giving COVID-19 vaccines to long-term care residents and health care workers by the end of this month and has moved people aged 65 and older into the next group to receive the shots.

Democratic Gov. Laura Kelly on Thursday announced new details about the order in which her state’s residents will be eligible for inoculations, and making people aged 65 to 74 years an earlier priority was the biggest shift.

The state’s previous plan had that age group getting theirs after people in “congregate” living, such as state hospitals, shelters for the homeless, and prisons.

The governor’s comments last month that prisoners would get vaccinated relatively early prompted scathing comments from the Kansas Republican Party and leaders in the GOP-controlled Legislature.

But Kelly said during a Statehouse news conference, “This is all based on science.”

The American Medical Association&nbsp;<a href="https://www.ama-assn.org/press-center/press-releases/ama-policy-calls-more-covid-19-prevention-congregate-settings" target="_blank">adopted a policy</a>&nbsp;in November calling for prisons to have priority in getting vaccines, with one board member calling it among the steps vital to stopping the spread of the virus.&nbsp;<a href="https://justicelab.columbia.edu/sites/default/files/content/COVID_Vaccine_White_Paper.pdf" target="_blank">A report in December</a>&nbsp;from professors of medicine at Yale, Columbia and the University of North Carolina called for giving inmates vaccines early, saying outbreaks in prisons and jails “spill over to the community.”

“My administration will make our decisions based on what the doctors, the public health experts and the science tells us, not the politics,” Kelly said.

Kelly released the updated vaccine rollout plan, with a chart designed to make its five phases easier to follow, amid concerns that the process was moving too slowly.

On Monday, the Centers for Disease Control and Prevention ranked Kansas last among states for its inoculation rate per 100,000 residents. That was no longer the case in&nbsp;<a href="https://covid.cdc.gov/covid-data-tracker/#vaccinations" target="_blank">a CDC report</a>&nbsp;Thursday, with five states trailing Kansas.

The CDC’s data said 36,538 vaccine shots had been given in Kansas, or 1,254 per 100,000 residents. Kelly told reporters the actual figure is 45,872, or almost 26% higher and 1,575 for every 100,000 residents.

Dr. Lee Norman, head of the state Department of Health and Environment, said he believes the true number of shots given could be nearly twice Kelly’s figure, about 90,000.

“People are not sitting on the vaccines and keeping it in storage,” Norman said. “They’re giving it into people’s arms.”

The state’s revised vaccine distribution plan shows that the second group to get shots also will include critical workers including firefighters, law enforcement officers, meatpacking employees, grocery store workers, teachers and child-care workers.

The third phase includes people aged 16 to 64 with medical conditions that put them at severe risk if they are infected with COVID-19, including cancer and Type 2 diabetes, as well as workers in critical industries such as information technology, aviation, transportation and government.

People with less severe health problems are included in the fourth phase and the fifth and final group includes everyone else.

Counties with unused vaccine doses after inoculating all health care workers and long-term care residents who wanted to be vaccinated were supposed to shift spare doses to counties in need of more. But the process is paperwork heavy, and some counties just went ahead and vaccinated people in lower priority groups, said Dennis Kriesel, executive director of the Kansas Association of Local Health Departments.

He said the state should have moved quicker to explain the phases of the rollout, noting that people are desperate for information. He got one call from a woman in her 70s with lymphoma who wanted details because she hadn’t been able to see her grandchildren in a year.

“We didn’t need the vaccines to be ready to set our priorities,” he said. “We could have set the priorities months before that.”

Sedgwick County health director Adrienne Byrne said she has been getting emails from people with chronic illnesses who have hardly left their homes since March.

She said the county has administered about half of the 10,400 doses it has received and noted that the process is speeding up. A massive clinic has been set up at an arena, with staff capable of vaccinating 50 people every 15 minutes.

“Some of them are crying with relief,” she said. “I heard one gentlemen say to the person who gave him the vaccine: ‘You may have just helped save my life.’”

When it comes to getting tested for COVID-19, drive-thru testing sites have popped up all over the country. These sites have been effective at keeping the medical staff and the occupants in the vehicle safe while increasing the number of tests in a shorter amount of time due to the efficiency of the drive-thru process.&nbsp;&nbsp;&nbsp;&nbsp;

County Health Educator Katelyn Sigler advises anyone experiencing fever, fatigue, or shortness of breath to get tested for coronavirus. The Barton County Health Department has a drive-thru clinic with nurses staffed in their mobile unit. She says the number of people using the drive-thru has recently seen an uptick.&nbsp;&nbsp;&nbsp;

“For example, on Monday, we had 32 get tested, and I would say that’s above average. Our positivity rate was at 40%, which is still pretty high,” explained Sigler.&nbsp;&nbsp;&nbsp;

Sigler says there is no cost for the nasal swab test, and results are coming back, on average, within 48 hours.&nbsp;&nbsp;&nbsp;&nbsp;

“We’re asking that everyone fill out the forms before they come because it makes it quicker for the nurses and for them to come and go. We have COVID testing forms on the health department page on the Barton County website. Those can be printed and filled out and bring them to their visit, or they can email it after filling it out as it is a fillable form.”&nbsp;&nbsp;

Drive-thru testing is available weekly on Monday from 1 p.m. to 3 p.m., Wednesday and Friday from 9 a.m. to 12 p.m. at the health department. These sites have been effective at keeping the medical staff and the occupants in the vehicle safe while increasing the number of tests in a shorter amount of time due to the efficiency of the drive-thru process.&nbsp;&nbsp;&nbsp;&nbsp;County Health Educator Katelyn Sigler advises anyone experiencing fever, fatigue, or shortness of breath to get tested for coronavirus. The Barton County Health Department has a drive-thru clinic with nurses staffed in their mobile unit. She says the number of people using the drive-thru has recently seen an uptick.&nbsp;&nbsp;&nbsp;“For example, on Monday, we had 32 get tested, and I would say that’s above average. Our positivity rate was at 40%, which is still pretty high,” explained Sigler.&nbsp;&nbsp;&nbsp;Sigler says there is no cost for the nasal swab test, and results are coming back, on average, within 48 hours.&nbsp;&nbsp;&nbsp;&nbsp;“We’re asking that everyone fill out the forms before they come because it makes it quicker for the nurses and for them to come and go. We have COVID testing forms on the health department page on the Barton County website. Those can be printed and filled out and bring them to their visit, or they can email it after filling it out as it is a fillable form.”&nbsp;&nbsp;Drive-thru testing is available weekly on Monday from 1 p.m. to 3 p.m., Wednesday and Friday from 9 a.m. to 12 p.m. at the health department. Barton County coronavirus update: 163 active cases

California on Wednesday announced the nation’s second confirmed case of the new and apparently more contagious variant of the coronavirus, offering a strong indication that the infection is spreading more widely in the United States.

Gov. Gavin Newsom announced the infection found in Southern California during an online conversation with Dr. Anthony Fauci, head of the National Institute of Allergy and Infectious Diseases.

“I don’t think Californians should think that this is odd. It’s to be expected,” Fauci said.

Newsom did not provide any details about the person who was infected.

The announcement came 24 hours after word of the first reported U.S. variant infection, which emerged in Colorado. That person was identified Wednesday as a Colorado National Guardsman who had been sent to help out at a nursing home struggling with an outbreak. Health officials said a second Guard member may have it too.

The cases triggered a host of questions about how the version circulating in England arrived in the U.S. and whether it is too late to stop it now, with top experts saying it is probably already spreading elsewhere in the United States.

“The virus is becoming more fit, and we’re like a deer in the headlights,” warned Dr. Eric Topol, head of Scripps Research Translational Institute. He noted that the U.S. does far less genetic sequencing of virus samples to discover variants than other developed nations, and thus was probably slow to detect this new mutation.

The two Guard members had been dispatched Dec. 23 to work at the Good Samaritan Society nursing home in the small town of Simla, in a mostly rural area about 90 miles outside Denver, said Dr. Rachel Herlihy, state epidemiologist. They were among six Guard members sent to the home.

Nasal swab samples taken from the two as part of the Guard’s routine coronavirus testing were sent to the state laboratory, which began looking for the variant after its spread was announced in Britain earlier this month, Herlihy said. Samples from staff and residents at the nursing home are also being screened for the variant at the lab, but so far no evidence of it has been found, she said.

The Colorado case announced Tuesday involves a man in his 20s who had not traveled recently, officials said. He has mild symptoms and is isolating at his home near Denver, while the person with the suspected case is isolating at a Colorado hotel while further genetic analysis is done on his sample, officials said.

The nursing home said it is working closely with the state and is also looking forward to beginning vaccinations next week.

Several states, including California, Massachusetts and Delaware, are also analyzing suspicious virus samples for the variant, said Dr. Greg Armstrong, who directs genetic sequencing at the Centers for Disease Control and Prevention. He said the CDC is working with a national lab that gets samples from around the country to broaden that search, with results expected within days.

The discovery in Colorado has added urgency to the nation’s vaccination drive against COVID-19, which has killed more than 340,000 people in the U.S.

Britain is seeing infections soar and hospitalizations climb to their highest levels on record. The variant has also been found in several other countries.

Scientists have found no evidence that it is more lethal or causes more severe illness, and they believe the vaccines now being dispensed will be effective against it. But a faster-spreading virus could swamp hospitals with seriously ill patients.

The discovery overseas led the CDC to issue rules on Christmas Day requiring travelers arriving from Britain to show proof of a negative COVID-19 test. But U.S. health officials said the Colorado patient’s lack of travel history suggests the new variant is already spreading in this country.

Topol said it is too late for travel bans.

“We’re behind in finding it. Colorado is likely one of many places it’s landed here,” he said. “It’s all over the place. How can you ban travel from everywhere?”

Colorado public health officials are conducting contact tracing to determine its spread.

Researchers estimate the variant is 50% to 70% more contagious, said Dr. Eric France, Colorado’s chief medical officer.

“Instead of only making two or three other people sick, you might actually spread it to four or five people,” France said. “That means we’ll have more cases in our communities. Those number of cases will rise quickly and, of course, with more cases come more hospitalizations.”

London and southeast England were placed under strict lockdown measures earlier this month because of the variant, and dozens of countries banned flights from Britain. France also briefly barred trucks from Britain before allowing them back in, provided the drivers got tested for the virus.

New versions of the virus have been seen almost since it was first detected in China a year ago. It is common for viruses to undergo minor changes as they reproduce and move through a population. The fear is that mutations at some point will become significant enough to defeat the vaccines.

South Africa has also discovered a highly contagious COVID-19 variant that is driving the country’s latest spike of cases, hospitalizations and deaths.

Highly infectious COVID-19 variant spreading in US

WASHINGTON (AP) — U.S. health officials believe the coronavirus mutation that set off alarms in parts of Britain is no more apt to cause serious illness or be resistant to vaccines than the strain afflicting people in the United States but it still must be taken “very seriously,” the government’s top infectious disease expert said Sunday.

Dr. Anthony Fauci endorsed the decision of U.S. officials to require negative COVID-19 tests before letting people from Britain enter the U.S. He declined to weigh in on whether that step should have been taken sooner. He said the variant strain is something “to follow very carefully” and “we’re looking at it very intensively now.”

He said: “Does it make someone more ill? Is it more serious virus in the sense of virulence? And the answer is, it doesn’t appear to be that way.” British officials are telling their U.S. colleagues it appears that the vaccines being rolled out will be strong enough to deal with the new variant but, Fauci said, “we’re going to be doing the studies ourselves.”

Fauci said the U.S. is at a critical phase of the pandemic, with the worst probably still ahead. He predicted the general population would be getting immunized widely by late March or early April — beyond the front-line workers, older people and certain other segments of the public given priority for the vaccines.

Fauci spoke on CNN’s “State of the Union.”

them require people to get two doses? And do they need two doses of the

immunized to continue wearing masks and practicing safe physical 
distancing until more is learned.

Is it safe for patients with
 autoimmune disease who take immunosuppressive medications to be

There's
 no doubt that we're going to have to look very carefully at data on 
both the safety and efficacy because that too can be impaired in 
immunocompromised individuals. The vaccines that are typically not 
recommended routinely for severely immunocompromised individuals are 
what we call attenuated viral vaccines, where we take the natural virus 
and make it weaker. And this is a very old way of making vaccines. But 
for example, our measles, mumps, and rubella vaccines are attenuated 
viral vaccines. None of the front runners for COVID-19 or SARS-CoV-2 are
 that type of vaccine. So I don't anticipate problems with safety ... 
but that will need to be studied going forward.

Do any of the manufacturers know if their vaccine is safe for people with HIV? From William Schaffner MD, medical director of the National Foundation for Infectious Diseases.

It
 probably won't hurt, but it could help. The second vaccine in line -- 
the Moderna vaccine -- included a small study of people with HIV 
infection and we expect to see that data shortly. But in the meantime, 
if people with HIV are offered the Pfizer vaccine they should take it.

 Vaccine providers will be able to bill insurance for a fee to 
administer the vaccine, but will not be able to charge you. They can 
seek reimbursement for uninsured patients from the Health Resources and

Q&A about COVID-19 vaccines as inoculations roll out

WASHINGTON (AP) — The U.S. added a second COVID-19 vaccine to its arsenal Friday, boosting efforts to beat back an outbreak so dire that the nation is regularly recording more than 3,000 deaths a day.

Much-needed doses are set to arrive Monday after the Food and Drug Administration authorized an emergency rollout of the vaccine developed by Moderna Inc. and the National Institutes of Health.

The move marks the world’s first authorization for Moderna’s shots. The vaccine is very similar to one from Pfizer Inc. and Germany’s BioNTech that’s now being dispensed to millions of health care workers and nursing home residents as the biggest vaccination drive in U.S. history starts to ramp up.

The two work “better than we almost dared to hope,” NIH Director Dr. Francis Collins told The Associated Press. “Science is working here, science has done something amazing.”

Early results of large, still unfinished studies show both vaccines appear safe and strongly protective although Moderna’s is easier to handle since it doesn’t need to be stored at ultra-frozen temperatures.

A second vaccine represents a ray of hope amid despair as the virus continues to spread unabated even before holiday gatherings that are certain to further fuel the outbreak.

The scourge has claimed more than 312,000 U.S. lives and killed 1.7 million people worldwide. New cases in the U.S. are running at over 216,000 per day on average. Deaths per day have hit all-time highs, eclipsing 3,600 on Wednesday.

California has emerged as one of the most lethal hot spots, with hospitals running out of intensive care beds and ambulances lining up outside emergency rooms in scenes reminiscent of the calamity around New York City last spring. California on Friday reported over 41,000 new cases and 300 more deaths.

When New York’s hospitals were in crisis, health care workers from across the country came to help out. This time, “there’s no cavalry coming” because so many hospitals are swamped, said Dr. Marc Futernick, an emergency room physician in Los Angeles.

And while health workers are enthusiastically embracing vaccination, authorities worry the public may need more reassurance to ensure more people get in line when it’s their turn.“Frankly if we don’t succeed in getting 80% or so of Americans immunized against COVID-19 by the middle of this 2021 year, we have the risk that this epidemic could go on and on and on,” Collins said.He is especially concerned that accurate information about the shots’ value reaches communities of color, which have been hard-hit by COVID-19 yet also are wary after years of health care disparities and research abuses.To try to help instill confidence, Vice President Mike Pence received the Pfizer-BioNTech shot on live TV Friday, along with Surgeon General Jerome Adams.FDA’s decision could help pave the way for other countries that are considering the Moderna vaccine, the first-ever regulatory clearance for the small Cambridge, Massachusetts, company. European regulators could authorize its use as soon as Jan. 6. Britain, Canada and a few other countries already have cleared the Pfizer-BioNTech shot, with a European Union decision due Monday.“What we want to always remember is one size does not fit all. We want to have options,” said Dr. Paul Duprex of the University of Pittsburgh.Moderna has about 5.9 million doses ready for shipment set to begin over the weekend, according to Operation Warp Speed, the government’s vaccine development program. Injections of health workers and nursing home residents continue next week, before other essential workers and vulnerable groups are allowed to get in line.Both Moderna’s and Pfizer-BioNTech’s shots are so-called mRNA vaccines, made with a groundbreaking new technology. They don’t contain any coronavirus – meaning they cannot cause infection. Instead, they use a piece of genetic code that trains the immune system to recognize the spike protein on the surface of the virus, ready to attack if the real thing comes along.Their development less than a year after the coronavirus first emerged set a speed record, but Collins stressed that shouldn’t worry people. The speed was due to billions in company and government investments paired with years of earlier scientific research, not any cut corners.“The rigor of the analysis of these vaccines is unprecedented,” Collins said. “We’re not done with this but hope is on the way, and the hope comes from this scientific brain trust that has pulled out all the stops.”Experts are hoping the two vaccines together will “break the back of the pandemic” when combined with masks and other precautions, said Dr. Arnold Monto of the University of Michigan, who chaired an advisory committee that publicly debated the shots’ evidence ahead of FDA’s decisions.The FDA’s main messages:--Both the new Moderna vaccine and the Pfizer-BioNTech shot require two doses several weeks apart. The second dose must be from the same company as the first.--In a study of 30,000 volunteers, the Moderna vaccine was more than 94% effective at preventing symptomatic COVID-19 in people 18 and older. It also strongly protected older adults, who are most vulnerable.--The inoculated can’t throw away their masks as it’s not yet clear either vaccine prevents silent, symptomless virus spread. But there was a hint that Moderna’s shot might provide some protection against asymptomatic infection.--The Moderna study uncovered no major safety problems. Like with the Pfizer-BioNTech shot, expect sore arms, fever, fatigue and muscle aches, which are signs the immune system is revving up.--Moderna’s study turned up no severe allergic reactions, like the handful reported with the Pfizer-BioNTech shots in Britain and Alaska. The vaccines’ ingredients aren’t identical. Still, after any COVID-19 vaccination, people should stick around for 15 minutes – or 30 minutes if they have a history of severe allergies – so if they do have a reaction, it can be treated immediately.--Both vaccines remain experimental, and the government is closely monitoring safety in case rare problems crop up.--Additional studies are needed to tell if the vaccine should be used by children and pregnant women.___
